See REPORTING-BUGS file in kernel souce for a suggested bug-report format

Information required
summarise the bug into a line or so
use the short summary as the subject of the email
have a more terse description of the bug
decoded oops
kernel version
kernel configuration
if you can describe how to reproduce the bug
any other relevant information

Email the report to the maintainer of that system
MAINTAINERS file
if you don't know who to send it to, sent it to the Linux Kernel Mailing list
